Why NAD+ Matters in Exercise Metabolism

NAD+ is the electron acceptor in glycolysis, the TCA cycle and fatty acid oxidation. Every ATP molecule produced aerobically depends on NAD+ cycling between its oxidized and reduced forms. It is also the required substrate for sirtuins and PARPs, which govern mitochondrial biogenesis and DNA repair. Intense exercise increases NAD+ turnover, and tissue NAD+ declines with age.

The Precursors: NR, NMN and Niacin

NAD+ itself is poorly absorbed orally, so supplementation uses precursors. Nicotinamide riboside (NR) has the largest human trial base and clear pharmacokinetic data. Nicotinamide mononucleotide (NMN) has growing human data and a regulatory status in the US that has shifted. Plain nicotinamide and niacin raise NAD+ as well and cost a fraction as much — a comparison the supplement market rarely foregrounds.

What the Performance Trials Actually Found

Human trials consistently show that NR and NMN raise blood NAD+ levels — that part replicates. Translation to performance is inconsistent. Several NR trials in older and middle-aged adults found no significant improvement in aerobic capacity or muscle function despite elevated NAD+. Some NMN trials in trained runners reported improved aerobic threshold at higher doses. The honest summary: the biomarker moves, the performance endpoint usually does not, and the trials that do show benefit are small.

Protocols in Practice

Typical reported use is 300–600 mg NR or 250–500 mg NMN once daily, taken in the morning, sometimes paired with a methyl donor such as TMG on the theory that NAD+ metabolism consumes methyl groups. Cycling is common but not evidence-based. Intravenous NAD+ infusions are marketed heavily, cost far more, and have essentially no controlled performance data behind them.

Safety Profile

NR and NMN have been well tolerated in trials up to a year at the doses above, with mild gastrointestinal effects the most common complaint. Neither is prohibited by WADA. Long-term safety beyond trial durations is not established, and the theoretical concern about NAD+ availability in the context of existing malignancy is unresolved rather than refuted.

Verification for Supplements, Not Just Peptides

NAD+ precursors are sold as dietary supplements, which means no pre-market potency verification. Third-party analyses have repeatedly found NMN products with content well below label claim. Look for a batch-specific certificate of analysis and a third-party testing program; treat a product without either as unverified.
